MY Solutions
Established in 2018, MY Solutions is committed to providing access to the highest quality of care to patients, offering personalized services including primary care, urgent care, and transitional care. We also provide comprehensive case management, preventative care, palliative care, and behavioral health and wellness visits.
Whether patients reside in the community or at a facility, expert management of acute and chronic conditions are addressed using the latest in evidence-based practice. Our community partners and providers work together to ensure our patients’ needs are met by building relationships and forging trust.
MY Solutions offers alternative services to patients based on their specific needs. We also collaborate with individual patients’ Primary Care Providers. For those who are in need of temporary care and unable to make it to their provider due to physical or cognitive conditions, in-home interim care is also offered. Our mission is to improve the health and well-being of the community.
Our providers specialize in Geriatrics, Psychiatry, Endocrinology, Cardiology, Primary Care, and Nephrology. We partner with Registered Dietitians, Licensed Mental Health Providers, and Leaders in Social Services to care for the overall health of patients.
MY Solutions has been a part of many community initiatives, including offering mental health counseling to healthcare workers during the COVID-19 pandemic.

Medina Yasin, ARNP, CEO of MY Solutions
With over 20 years in healthcare, Medina Yasin, ARNP specializes in implementing clinical innovations while providing quality healthcare and advocacy to the community.
Medina graduated from Molloy College with a Bachelor’s Degree in Nursing during which time she gained clinical expertise and extensive knowledge working in Critical Care, Hospice Care, and Case Management. Seeking to advance in her career, Medina then obtained a Master’s of Nursing from South University. One of her biggest accomplishments was transitioning displaced patients, often homeless, residing in skilled nursing facilities back into the community through the Medicaid Diversion Program.
In search of a flexible next step in her career, she sought the opportunity to combine her love for healthcare, community, and education by establishing MY Solutions. MY Solutions in collaboration with local and state universities offers scholarships for graduating high school seniors pursuing advanced education in healthcare. MY Solutions is committed to expanding opportunities for nurses and nurse practitioners in the community through our mentorship/apprenticeship programs for both nurses and Nurse Practitioners. It is through this program we are able to provide employment opportunities. Our goal at MY Solutions is to provide a platform for healthcare professionals while providing stability and encouraging a healthy work-life.
Medina is a wife, mother of four amazing children, philanthropist, and community activist.
