Founded in 2018, MY Solutions is dedicated to enhancing healthcare access by offering innovative staffing alternatives. Specializing in medical staffing for local, federal, state, county, and non-governmental entities, MY Solutions provides cutting-edge telemedicine and health assessment services in collaboration with community healthcare providers.
Services: Telehealth, In-Home Care, Assisted Facility Care, Primary Care, Endocrinology, Gerontology, Psychiatry, Nursing Services, and Case Management.
Whether patients reside in the community or at a facility, expert management of acute and chronic conditions are addressed using the latest in evidence-based practice. Our community partners and providers work together to ensure our patients’ needs are met by building relationships and forging trust.
MY Solutions offers alternative services to patients based on their specific needs. We also collaborate with individual patients’ Primary Care Providers. For those who need temporary care and unable to make it to their provider due to physical or cognitive conditions, in-home interim care is also offered. Our mission is to improve the health and well-being of the community.

With over 20 years of clinical expertise, Medina Yasin, ARNP, excels in implementing clinical innovations and advocating for those needing hands-on, intentional care. Driven by a passion for healthcare, public service, and education, Medina established MY Solutions as a flexible next step in her career. Medina graduated from Molloy College with a Bachelor’s Degree in Nursing, followed by a Master's in Nursing at South University, during which time she gained clinical expertise and extensive knowledge working in Critical Care, Hospice Care, and Case Management.

Medina is a wife, mother of four amazing children, philanthropist, and community activist.
